2. Issue Certificates for approximate age

Documents required to register under anecdotal birth under section 24

1. Hospital report/Grama Niladhari B-23 
2. Parent's marrage certificates.
3. Parent's birth certificates.
4. National Identity cards of parents and 2 copies.
5. A cpoy of the Helth Development Note.
6. Resident dwelling of Grama Niladhari (with approval of Divisional Secretary)
7. Blank result

• Mother,Father and child with the child on weekdays 8.30 am  shoud arrived between 3.00 pm.

Documents required to make a change in the father's seat in birth certificate or marrital status of the parents or any amendment to the gratitute of the parent (27A)

1. Original copy of the birth certificate wich should be revised.
2. Original father's birth certificates.
3. Original of the parent's marrage certificate.
4. Father's National Identity Card and a photocopy. 

Documents required for the birth of a birth certificate or sex (52-1)

1. Original copy of the birth certificate that should be amended.
2. A report on the abirth of a hospital.
3. Grama Niladhari's B-23 report and other documents related to birth.
4. Original of the parent's marrage certificate.
5. National Identity Card of the parent /guardian and its copies.

Documents required to correct the information of the mother in the birth certificate or the information of the informant.

1. Original copy of the birth certificate which should be revised.
2. Mother's original birth certificate.
3. Original copy of mother's marriage certificate.
4. Mother's National Identity Card and a copy thereof.

• Declaration should be submitted by the individual / mother / father / guardian

Documents required to change the name of a person below the age of 07 years.

1. Original copy of the birth certificate that should be amended.
2. Parent's written request.
3. An affidavit.
